New Patient Registration Forms
Please click on the link below and print all three pages which includes general patient demographics, pain diagram, and a current injury status/past medical history. Please fill out the documents prior to your appointment and bring with you to your initial evaluation visit. This will ensure that all patients are seen in a timely manner. Remember to still bring your insurance card to your appointment along with your doctor’s prescription unless you are self-referred.
Other tips to consider:
1. Bring comfortable/appropriate clothing (i.e. If we are seeing you for a knee injury, please bring a pair of shorts)
2. Bring a bottled water
3. Take your medication as prescribed by your doctor (i.e. If you are post-surgical, consider taking pain medication an hour or two before your appointment)
4. Please arrive at least 15 minutes early with your completed paperwork. This allows the receptionist time to enter your information into our system and review your insurance coverage and payment arrangements before seeing the physical therapist. We strive to be prompt with all of our appointment times. We value the time of our patients, as well as, the time schedule of the physical therapists.
5. As a courtesy, if you are unable to make your initial evaluation appointment, please provide us at least a 24 hour notice.
