Dr. Sean White, PT, DPT, OCS, CMDT

Owner

Sean is originally from Denver, CO. and received his Bachelor’s degree in Exercise & Sports Science with a double minor in Anatomy and Neurobiology  from Colorado State University in 1996.

Sean earned his Master’s degree in Physical Therapy from Gannon University in 1998 and recently completed his Doctorate of Physical Therapy from Utica College.  Sean earned a distinction as a board certified orthopedic clinical specialist (OCS) after completing a rigorous examination instituted by American Board of Physical Therapy Specialties (ABPTS).   Sean also completed specialty post-graduate training and certification from the McKenzie Institute of North America called Mechanical Diagnosis & Therapy (CMDT), originated in New Zealand and founded by Robin McKenzie.

Sean relocated to Northeast Indiana in 2002 where he worked as a clinical director for a national conglomerate before venturing into private practice in 2006 opening Peak Performance Orthopedic & Sports Physical Therapy, PC in Columbia City, IN.  Peak Performance later expanded the practice with a second location in Warsaw in 2008.

Sean is an active member of the American Physical Therapy Association (APTA), Indiana Physical Therapy Association (IPTA), Orthopedic speciality section of the APTA and member of the McKenzie Institute of North America.

 

Dr. Heather White, PT, DPT

Warsaw Clinic Director

Heather grew up on Blue Lake in Churubusco.  She attended Churubusco High School where she graduated in the top ten of her class. She then went on to receive her Bachelor’s degree in Health and Exercise Science and degree as a Physical Therapist Assistant in 2010 from the University of Saint Francis.

Heather recently graduated from the University of Findlay earning her Doctorate of Physical Therapy.

Heather currently resides in Fort Wayne.  She enjoys spending time with family and friends and golfing.

 

Shannon Johnson, PTA, BS

Physical Therapist Assistant

Shannon grew up and continues to live in the Kosciusko Community.  She received a Bachelors of Applied Health Science from Indiana University of Bloomington and went on to the University of Saint Francis in Fort Wayne for an Associates of Science in the Physical Therapist Assistant Program. Shannon is an experienced Registered Yoga Teacher and Therapist and has been teaching yoga for 15 years.  Shannon enjoys helping people connect with their bodies so that healing can take place.

 

Lindsey Bolen, PTA, BS

Physical Therapist Assistant

Lindsey was born and raised in Indiana. As a 2010 graduate from East Noble High School, she went on to continue her education at the University of Saint Francis. In 2013, Lindsey completed her PTA degree. She then went on to receive her Bachelors degree in Health and Exercise Science in 2014 from the University of Saint Francis.

Upon graduation, Lindsey practiced in an outpatient clinic in Florida for two years. She relocated back to Indiana in 2016 to continue her career.
Outside of work, Lindsey loves to stay involved in the community and keep active. She enjoys training and competing in mud runs and obstacle courses and completed her first Tough Mudder course in 2014. She also spends her time enjoying the company of her family and adventuring outdoors.
